At UW Health Partners, Patient Navigation is part of our patient-centered care experience. Our Heart and Vascular navigator is available free of charge for every patient referred to the Heart and Vascular Center. They are an advocate for you and will guide you through our system to ensure that your care is designed just for you.
Heart and Vascular Navigator
Our navigator is a registered nurse who is specially trained to provide support and guidance to help you and your families “navigate" through our health care system.
Our navigator, Laurie, has more than 20 years of healthcare experience and is dedicated to providing personalized and attentive care. She is committed to assisting you throughout your hospital experience.
What Does The Navigator Do?
The Navigators are experienced registered nurses dedicated to serving as a resource to you. They offer an individualized approach to help you better understand your care from the moment of referral to diagnosis, treatment, recovery and even through chronic care if needed. Working directly with the patient, the navigator serves as a personal guide to Heart and Vascular care. They provide services such as patient education, support and care coordination as well as assisting with referrals and transfers and coordinating care needs. If needed, the navigator can assess emotional, spiritual, financial and other needs and connect patients with the required assistance.
